We provide Telemedicine on our high-grade secure and private site called doxy.me. Telemedicine delivers convenient and easy access to your healthcare professional.

Telemedicine uses telephone and video communication as a substitute for in person visits.

Criteria for Telemedicine appointments for our family practice patients

To be able to use Telemedicine as one of your health care avenues at our office, the following criteria must be met:

• All patients must sign an Electronic Communication Consent Form for your medical chart. (Please call the office to have a self populating form emailed to you)

• All patients must complete their yearly Mental Health Plan.

• All patients must have their pap’s and mammogram’s up to date.

• All contact information must be up to date: address, telephone, cell phone, email.
